Important Forms & Documents
Forms that require a signature must be printed and submitted in person, by fax, by mail, or scanned and emailed as an attachment. All forms must be complete and contain appropriate signatures or they WILL NOT be processed.

| Due three (3) terms before anticipated graduation date. Once approved, this form will indicate the courses required before a student’s degree can be awarded. Please allow 8-10 weeks for processing. |
Used to add or drop a course from a student’s registration ONLY when registration throught Self-Service is not possible. You may use these forms if it is still within the quarter’s allotted add/drop time frame. Please reference the current academic calendar for Add/Drop dates. May be processed immediately in person. | |
Course work to be completed independently in consultation with an instructor. Directed Study cannot be used for any labs, studio/performing arts, or when similar course is being offered within the same quarter. Please allow 5 business days for processing. | |
Request to have a Degree Auditor review a student’s academic progress toward degree completion. Please allow 10-15 business days for processing. | |
| This form is to be completed by seniors who DO NOT plan to graduate in the current academic year. All Seniors must complete either the Applicaiton for Gradation or the Deferral Form. Please allow 1-2 business days for processing. |
| Used to establish course equivalencies for articulation. Must be submitted and approved before a student takes any course at another institution. Please allow 10 business days for processing. |
Given for circumstances beyond a student’s control, and only if at least 75 percent of the coursework has been completed (must be approved by instructor). A student requests an “I” grade by completing and leaving the form with the instructor. Default grade will automatically be issued if no other actions are taken before the time extension limit. | |
To request academic transcripts be sent through the mail or picked up from our office. Students must clear outstanding financial obligations to the University before official transcripts can be issued. Regular Processing: $5.00 for (5-10 working days) Rush Processing: $10.00 (48 hours) Overnight Processing: $50.00 | |
Unspecified non-degree status may be available to students who are not interested in seeking a degree at La Sierra University. There is no guarantee that these courses will transfer into a degree/ pre-professional program or back to the student’s home university. Please allow 1 week for processing. |
