If you are a student who is currently registered with the Office of Disability Services for this quarter, you may use this form in order to take your tests with certain approved accommodations in the Learning Support and Testing Center in Suite 100 in the basement of La Sierra Hall if you so desire.

If by some reason you are unable to come to the LSTC personally to pick up an ODS Test Proctor Form, you may print out a copy to fill out.

Please make sure that all necessary information is completed by you and your professor. Any missing or ignored information may void your Proctor Form altogether. Getting this form to your professor, then to the Student Services Assistant at the Front Desk of the LSTC/ODS is your responsibility, so be diligent and deliver your form in a timely manner.

*All ODS Test Proctor Forms must be completed and turned into the LSTC at least three (3) days prior to your testing date. This will enable the LSTC to be prepared to fulfill your approved accommodation requests to the best of our abilities.
