If you experience any difficulty in opening the form, please first open "Adobe Reader" on your computer, then click on "File" and then "Open." Input the following complete address into the filename box: http://goo.gl/T0gvS
Once you have completed the form, save it and e-mail the .pdf and supporting documents to the appropriate contact(s) listed below.
If a student wishes to use one or more of these classes listed on the equivalency chart., she or he should email such a request to the MBA Advisor indicating which class(es) is/are suggested for waiver of the La Sierra requirement(s).

If a student wishes to request a waiver based on previous coursework that does not appear on the equivalency chart, she or he will need to make a formal request of the program coordinator in that area, and provide appropriate supporting documentation (syllabus of the course(s) being requested as waivers, and copy of a transcript showing units and grade earned), and copy the MBA advisor on the request. Below is the name and contact information for the program coordinators and for which areas each is responsible:

Please note that a student must have earned a grade of C or above in any class she or he wishes to have considered for waiver.

If a student wishes to have previous graduate coursework considered as units to be transferred into the 48 degree units of the MBA program, that student will need to make a formal request of the program coordinator in that area and copy the MBA Advisor, by writing a request and providing appropriate supporting documentation for each class being considered.Please refer to the Waiver and Transfer information sheet for complete details by CLICKING HERE.
