Practical Training (OPT)
If you are finishing your studies this coming June
and would like to apply for Optional Practical
Training, you must come and see Natan in the OISS.
You can make an appointment by calling the front
desk at x2100. ONLY undergraduate and graduate
students qualify for OPT.
OPT applications must be filed before the last of
classes or June 11th.
In order to apply for OPT, you will have to follow
the guidelines listed in the OPT Application Form,
available outside the OISS. You will need:
- a valid passport & I-94
- a valid I-20
- two pictures (must follow INS guidelines!)
- check for $120
- form I-765
- verification of studies signed by your academic
advisor/graduation counselor
Taxes
Are Due April 15h
April 15th is fast approaching, and if you haven't
filed your taxes yet, you might want to do so as soon
as possible.
The Office of International Student Services
has arranged for someone experienced to help you
with your taxes for a minimal fee. If you are single,
the fee will be $25. If you are married and you're
filing jointly, the fee will also be $25. If you are
married and you are filing separately, the fee will
be $50. Here is what you will need to do:
- Obtain your W-2
form from the Payroll Office. This should have
been mailed to you if you worked at La Sierra University
during 2003. If you have not received yours yet, contact
the Payroll Office at 909.785.2034. If you are a transfer
student and worked at your previous institution during
2003, you should also get a copy of your W-2 from
that institution.
- Get the 2003 tax check list from the OISS. The check
list is available outside the office, on the corner
with all the other forms.
- Fill out page 5 of the 1040-NR-EZ form, also available
outside the OISS.
- Return all the documents and forms requested in
the 2003 Tax Check list in an envelope, along with
the fee, to Natan in the OISS.
Your documents will be available for pick up after
ten (10) working days of you turning them in into us.
REMEMBER - You must file your taxes
before April 15, 2004! Please do not wait until
the last day to do your taxes.
