Application
for student teaching must be submitted two
months prior to the quarter that you plan to begin.
The information must be available so that processing can be completed one
month prior to the date you plan to begin student teaching. Documentation must be provided for each of
the items required and the forms must be completed with the appropriate signatures.
When this form is complete, turn it in to the secretary in the Curriculum
and Instruction Department for processing.
See the Credentials Officer to verify the following:

· | CBEST (California Basic Education Skills Test) must be PASSED. | q |
· | Certificate
of Clearance MUST be on file with the credential officer. (The Certificate
of Clearance is a California Department of Justice and FBI clearance.
You will NOT be allowed
to do student teaching without this. It

| Elementary: State approved Liberal Studies Major or MSAT if you are not a liberal studies major but plan to teach Elementary school | ||
