Please print them out, complete them legibly in blue or black ink, and submit the hard copy to the Office of the Registrar. All forms must be complete and contain all appropriate signatures in order to be processed. Incomplete forms WILL NOT be processed. You may submit these forms in one of the following ways:

• Transcript request form -- $5.00 for regular processing (5-10 working days) $8.00 for rush and fax processing (48 hours, then picked up or mailed)
• Academic Credit Evaluation Request – Please allow 2 weeks for processing
• Off-Campus Registration Request Form – Please allow 2-3 weeks for processing
• Application for Graduation – Please allow 4-6 weeks for processing. Due 3 terms before graduation.
• Unspecified Student Registration Form – CANNOT be mailed OR faxed. Must be submitted in person.
