NAD Paper Application

You are required to have 3 recommendations filled out by the following people: a church pastor or elder, an employer, and a teacher. Please fill out your personal information and submit forms to your desired recommenders. Have the recommender return the completed recommendation form to the Missions office.

The Volunteer Ministries Agreement is ONLY for volunteers serving in the Guam-Micronesia Mission. This includes the islands of Chuuk, Ebeye, Guam, Kosrae, Marshall Islands, Palau, Pohnpei, Saipan, and Yap.

After completing ALL the application forms, please email us at missions@lasierra.edu for an appointment to turn the completed forms in.
