MEDICAL RELEASE FORM Instructions The Medical Treatment Authorization form is for students traveling on a university sponsored trip, to complete and keep in possession of the university leader. This form is to be used in the event of an emergency situation where the injured person may be unable to give permission or sign for medical treatment.
MEDICAL HISTORY FORM Instructions The Medical History form is for students traveling on a university sponsored trip, to complete and keep in possession of the university leader. This form is to accompany the Medical Treatment Authorization form in the event of an emergency situation where the injured person may be unable to give permission or sign for medical treatment.
