The following Records forms are to be printed ONLY. These forms CANNOT be completed electronically or submitted online. Please print them out, complete them legibly in blue or black ink, and submit the hard copy to the Office of the Registrar. All forms must be complete and contain all appropriate signatures in order to be processed. Incomplete forms WILL NOT be processed. You may submit these forms in one of the following ways:
