  1. If you were/are under the care of an MD for an injury, that particular MD needs to perform your physical or at least clear or not clear your injury.

  2. The paperwork needed to be submitted for all student-athletes is a pre-participation physical exam (please choose the correct form if you are a returner or if you are a first year student-athlete at La
Sierra) and the Insurance Forms (packet).

  3. All paperwork needs to be completed in black ink and all originals go to the Head Athletic Trainer.

  4. Fall sports due date for all paperwork is August 12, 2009; Winter sports due date is October 1, 2009 and Spring sports due date is October 31, 2009

  5. Physical must be performed by an MD or a DO. Campus Health Services will be charging $75 for athletic physicals and must have a scheduled appointment to have a physical if done at CHS. The SPORT Clinic in downtown Riverside conducts physicals also at a cost of $20-$25. Monday Friday 7a-8a and 12p-1p. These are based on a first come first serve.

  6. All freshman/transfer student-athletes as well as returning student-athletes need to submit to Campus Health Services an up-to-date immunization report and should also have a current PPD test which has been done in the last 12 months.  If you do not have a current PPD within the past year, you will not be cleared for practices and/or competitions. If you are planning on staying in the dorms either at the beginning of the season or during the school year, it is highly recommended that you get the Meningitis shot.

  7. All student-athletes must submit a copy of your physical (original form to the Athletic Trainer) to Campus Health Services to prevent holds on your account.

  8. If you have questions regarding these last two statements feel free to contact Campus Health Services (951)785-2200.
